El sobreviviente necesitará información que incluye una dirección de correo electrónico no asociada con el abusador; información de contacto del titular de la cuenta; comprobante del abuso cubierto y una declaración jurada para las líneas de los dependientes que el sobreviviente también desea que sean separadas.
